| تقنية التحضير في الوجوه التجميلية الخزفية الصرف THE EXTENDED TECHNIQUE After the Great Succes Of SADEKLY's Thread تقنية التحضير في الوجوه التجميلية الخزفية الصرفة تقنية التحضير في الوجوه التجميلية الخزفية الصرفة I wanted To Add some Good quality Photos and more details on the preparation procedure so I preferred To open a new Thread I hope this will be interesting for you. For indications please refer to SADEKLY's thread (link above) I just want to add one that can be considered as the most important indication for this technique: It is Restoring the central incisor preeminence for aesthetic reasons, this can be corrected with some composite but it isn't the best choice. Here is the procedure: First Of all we will make a Preparation Guide, so we make a print of patient teeth then a WAX-UP trying to mimic the final situation and form of the teeth. then after validation by the patient we make a vestibular print with some silicone material and we cut it horizontally (this will help us to see if preparation is sufficient and that we have nearly 0.6mm thickness) here is a photo Guide in Place: Image5.jpg after trying the cutting guide you can start the inter-proximal preparation : Image6.jpg then the vestibular preparation by making longitudinal strips then joining them : Image7 [640x480].jpg Image8 [640x480].jpg Then we verify the final thickness with the silicon guide : Image9 [640x480].jpg we'll end that by preparing the incisal edge : Image10.jpg |

| Great job Bassam, thanks a lot for this wonderful presentation and explanation.. Well, I would like to add a point regarding the finishing line at the lingual (palatal) aspect of the prepared tooth, in addition to some points regarding preparation in general: Technique Tips - On average, allow for a minimum of 1 mm of porcelain. A reduction greater than 1 mm may be required in areas masking severely discolored enamel -Allow for veneer thickness of approximately 0.8 mm labially with incisal covering -Proximal margins are completed labial to contact point. Preserve contacts wherever possible. It may be necessary to prepare through the proximal contact point when clinical conditions dictate (such as slice prep), interproximally to obtain proper emergence profile -Provide a definite finish line for the technician with a chamfer lingual preparation and a 0.5 mm labial gingival finishing area -Use medium grit, round-ended diamond bur to remove uniform thickness of facial enamel by joining the depth-cut grooves Incisal Preparation - Overlap of incisal edge is recommended - Long bevel lingual preparation - Lingual butt margin provides greatest strength as butt shoulder - 0.5 mm labial gingival finishing line - All line angles rounded, preparation smooth Interproximal Extension - Using round-ended diamond bur, extend the interproximal preparation from the gingival chamfer into the interproximal space. NOTE: Eliminate all discolored enamel into the interproximal - i.e., elbow prep Lingual Reduction to Increase Length - Reduce tooth surface in stress bearing areas to provide bulk of porcelain for function. - Angle diamond bur to eliminate undercuts and establish deep chamfer on lingual aspect of preparation |

| thanks Wesam good schemes : but I'm waiting to see somebody doing Horizontal Depth Cuts (there is a technical problem here) because you have to do them vertically(with a round Burr for sure this is less secure and control isn't the finest) while in the conventional technique you rely on the Burr thickness so it accelerates the work and we are less likely to enter the pulp.
