The text is clearly written and the chapters are well-illustrated. There is also a unique feature: several hundred evidence-based tables. These tables allow the reader to reach conclusions on the basis of data.” — New England Journal of Medicine “Is it different from the existing textbooks of surgery? Did the authors achieve their principal goal of providing an evidence-based approach to surgical decision making? Would I recommend it to the surgical residents in my program?…the answer to all these questions is yes. The ambitious undertaking of creating an evidence-based textbook of surgery has largel succeeded and I heartily recommend it.” — Journal of the American Medical Association “During a period of numerous adventures into the textbook arena, this is different in focus and quality. The 130 contributors constitute an excellent group of young surgeons, who, along with the editors, have stayed the course with this interesting format. The reader is taken through 62 case histories step by step and is asked to interpret a broad range of medical data. Most of the clinical problems are accompanied by illustrations, which include cardiographs, radiographs, clinical and pathological photographs. In addition to detailed questions and answers, a commentary is provided that highlights the important issues of each case, as well as a guide to further topics of study. The aim of any undergraduate curriculum is to produce graduates who have the knowledge and skills required to enable them to become competent members of the medical fraternity. Understandably, upon qualification these attributes will be raw and untried, but we expect them to be able to show that they possess what it is required of doctors in their first few years after graduation. What is ‘required’ in a clinical situation is often not obvious to the student and hence the reason for this book. The book represents a departure from the traditional formula of systems and diseases, and an exhaustive review of each subject. Rather the authors have selected what they believe to be a number of important and, for the most part, relatively common medical and surgical scenarios and approached each from the perspective of the practicing clinician presented with a clinical problem. Whilst the book has been written primarily for undergraduates it describes clinical problems as they will be managed by recently qualified doctors. It is not the purpose of this book to challenge or compete with standard reference material, rather it represents an important adjunct to the learning process of clinical reasoning, rather than provide the knowledge that must accompany it, although an considerable amount of factual material is to be found within its pages. Problem solving is a vital component of medical education, and while it can be taught well at the bedside and in small group tutorials, there are few books that help with this style of learning. http://rapidshare.com/files/1112894/Clinical_Problems_in_General_Medicine_and_Surgery.rar pass: lekar http://mihd.net/8xj9m7 pass: lekar http://z02.zupload.com/download.php?file=getfile&filepath=11122 http://rapidshare.com/files/3207196/Clinical_Problems_in_General_Medicine_and_Surgery.part1.rar http://rapidshare.com/files/3219477/Clinical_Problems_in_General_Medicine_and_Surgery.part2.rar http://depositfiles.com/files/372056 http://depositfiles.com/files/372062 -------------- Surgical Anatomy The Embryologic And Anatomic Basis Of Modern Surgery ![]() Product Details »Book Publisher: McGraw-Hill Professional Publishing (16 August, 2004) »ISBN: 9603990744 »Book author: John E. Students cannot know each of these subjects in as much detail as they did during their individual attachments and they need to concentrate on important topics rather than minutiae. Whereas existing textbooks cover many topics very well and in great detail, they are lengthy and difficult to read in a short space of time; some subjects are poorly covered and give no real understanding of practical management and logical thought processes — the exact requirements for passing finals. In this second edition, each chapter has been extensively rewritten and updated with the help of experts in their respective subjects. http://mihd.net/tu73fm Password: Lekar or: http://rapidshare.com/files/2810380/...In_Surgery.rar or: http://depositfiles.com/files/366547 or: http://www.icefile.net/index.php?pag...In_Surgery.rar |

| Selective Sentinel Lymphadenectomy for Human Solid Cancer (Cancer Treatment and Research Product Details »Book Publisher: Springer (21 March, 2005) »ISBN: 0387236031 »Book author: Stanley P.L. Leong, Yuko Kitagawa, Masaki Kitajima »Amazon Rating: Book Description: In human solid cancer, the lymph node (LN) status is the most important prognostic indicator for the clinical outcome of patients. Recent developments in the sentinel lymph node (SLN) concept and technology have resulted in the application of this revolutionary approach to define the first draining or SLN to which the cancer may have metastasized. The underlying thesis in solid cancer biology is that metastasis generally starts in an orderly progression, spreading through the lymphatic channels to the SLN in the nearest LN basin. Thus, the logical approach is to harvest that specific SLN for thorough analysis. Because a tumor-free SLN is usually associated with a negative residual LN basin, a negative SLN is an excellent indication that micrometastasis has not occurred in the regional LNs. When the SLN is involved, it is unknown whether or not metastasis is limited only to the SLN or if the disease has spread to the remainder of the nodal basin. For this reason, if an SLN is positive, a complete lymph node dissection is recommended. Therefore, selective sentinel lymphadenectomy (SSL) should be considered as a staging procedure so that patients with negative SLNs (about 80%) may be spared an extensive LN dissection. Malignant melanoma has been proven to be the most ideal tumor model to study the role of SLN. Subsequently, SSL has been applied to breast cancer, colon cancer and other types of solid cancer. The multidisciplinary approach encompassing the surgeon, nuclear medicine physician, and pathologist is the key to such a successful procedure. Such a team can be formed readily with appropriate training. Beyond the technical aspects of harvesting the SLN, the implication of micrometastasis remains to be defined. Because the follow-up of melanoma and breast cancer patients after SSL is crucial, ongoing clinical trials are in progress to determine the biological and clinical significance of SLNs. Although the concept of SLN is viable in other types of cancer, such as gynecological and gastrointestinal, the technical aspects of the procedure need to be perfected and verified. The most exciting possibility of SSL is that it will lead to early diagnosis of micrometastasis in regional LNs. Early diagnosis makes it useful as a clinical staging procedure, and opens up new opportunities to study micrometastasis and its evolution within the SLNs. Examining the multifaceted aspects of micrometastasis, such as differentiation of different clones with respect to the primary tumor, acquisition of adhesion molecules, and host interaction with the microscopic tumor, will shed new light on the biology of early metastasis. New molecular and genetic tools may be used to dissect the mechanisms of lymphatic and hemotogenous routes of metastasis. If such mechanisms can be understood, new therapeutic advances may be developed to prevent the process of micrometastasis. Rather than targeting larger tumor burdens such as Stage IV disease, targeted adjuvant clinical trials can be developed for high risk patients following definitive surgical resection. SSL is a standard staging procedure for patients with melanoma and is rapidly evolving into a standard procedure for breast cancer as well. |
