Case Report :

A 5 year old boy presents with a 1 day history of facial rash.This has become more prominent and extensive today and it appears to be red spots and macules on his upper arms...
The child is otherwise well and has a normal appetite and afebrile
,and has been exposed to many children at his kindergraten

NOW :

1- Put DD first

Again :
1- DD :
1- Erythema infectious
2-Erysipelas: an important DD here
this presents with well-demarcated facial erythema with-swelling and pain , BUT the striking symmetry facial rash in this child and lack of systemic symptoms make this diagnosis unlikely..........
3-SLE : the classical butterfly rash is seen with SLE , and the typically other systemic features are present concomitantly ..........
4-Acute contact dermatitis : the lack of vesicles in the erythematous eruption along with its striking symmetry make this diagnosis unlikely .....
5-Measles
6- Rubella
7-Drug eruption

2-The diagnosis is Erythema infectious or the fifth disease

3- It is caused by parvovirus B19

It is also known as Slapped Cheeks
It typically causes outbraks in spring with most cases occuring in children aged between 2 to 10 years

Some key features of EI are as follows :
-The incubation period is 4-14 days and the virus is transmitted by droplet aerosol
-The characteristic rash usually develops after MILD non-specific symptoms such as corzya and headache
-Confluent erythematous edematous plaques on the malar eminences appear first before a maculopapular or lace- like rash spreads to involve trunk, neck and limbs over subsequent 1 to 4 days
-Acute symmetrical polyarthritis involving the small distal joints of the limbs is the primary manifestation of B19 in adults , 10% of children also have joint symptoms

Diagnosis can be confirmed by the demonstration of raised Ig M to human parovirus .....


However the classic progression from slapped cheeks
to lacy eruption in extremities in the well-child lead you to EI


THIS IS BENIGN CONDITION SO SYMPTOMATIC TREATMENT IS ALL THAT IS USUALLY REQUIRED

السبب:طفح بقعي حطاطي على الوجه و الجذع بيطلع بعد ما تكون راحت الحرارة و الاعراض التنفسية العلوية

Dearling lune ,
measela and rubella are important DD for this disease but you must try again to know the right diagnosis !

as i tell u concentrate on the relationship between the two images ,

ما في علاج نوعي لأنو الانذار جيد و المناعة دائمة
ما بعرف إذا اللي قلتو صح بانتظار الرد
