The First Short Case:

This patient has CLL,
and two days ago he developed this rash

1-What is your diagnosis?
2-How would you treat him?

1-Herpes Zoster
2-Acyclovir
800mg 5 times/d for 7-10 d may shorten time to complete lesion scabbing, healing, and cessation of pain, may reduce duration of new lesion formation

Dear brother mm ,

I want more accurate diagnosis

and you dont explain how you will give acyclovir -i mean orally or IV in this case-

its acute herpese zoster in immunosupresent patient

The dose of acyclovir:
PO (adults) 800 mg q4h for 7-10 days
PO ( children) 250-600 mg per m2 qid

my answers :

1- herpes zoster.

2- acyclovir

the answers are :
1- this patient has disseminated herpes zoster

i want you to concentrate on his underlying disease CLL ,that is a predisposing factor to severe or disseminated herpes zoster

2- in immunocompromised or malignancy patient we use IV
Acyclovir 10 mg /kg q 8 hrs for 7-10 days


NB:
we give IV acyclovir in these cases

-in complications of herpes zoster
-atypical cases or in immunocompromised patients

and as mm said when we use it within the first 48 hrs ,
-the patients experience less pain
-faster resolution
-and may prevent recurrent outbreaks


and here is another image to disseminated zoster, note the absence of grouping of lesions that we see in typical zoster
